The hardest mode available, usually.
If it’s too hard I can usually drop it to make it easier, but if it’s too easy you’ve generally already cleared content before you can make it harder.
It’s so ridiculously hard, one shan’t even make the effort!

If we are talking about interactive fiction then easy/normal/cheat modes i like to read the story without micro managing every option.
If we are talling about video games then the hardest difficulty there is, if you play at hard from the start it become normal for you so to speak and challenge is fun.
Easy mode / story mode / cheat mode / god mode
