It’s either probably because:

  1. You forgot to include the
    <script src="../smPlugin.js"></script>
    <script src="../smPluginMenuAddon.js"></script>
    lines to the index.html
  2. You forgot to include the
    smPlugin.js
    smPluginMenuAddon.js
    files to the /web/ directory
